PRE-SCREENING [required]
1. All Visitors (Contractors, Service Providers and other Essential Visitors) must be screened and scheduled with Operations 24 hours prior to their arrival to be authorized as an “Approved Visitor”

2. Step 1 – to be completed by Visitor’s LJI Contact and emailed to ops@lji.org 24hrs prior to visitor’s arrival;

• Reason why visit to the building is essential
• Visitors first and last name(s)
• Company and contact information (Mobile number and email)
• Name and phone number of Visitor’s LJI contact and back-up LJI contact who will be responsible for escorting
• Date, arrival time and duration of Visitor(s) in the building
• Define any building services needed by the Visitor (Facilities, EH&S, Operations or DLAC)

3. Step 2 – to be completed by the Visitor and emailed to ops@lji.org directly by the Visitor’s LJI contact 24hrs prior to arrival [this can be included with the Step 1 portion];

• Where is your home base?
• Where have you been in the past 14 days? Be specific – Business name, type of business and location
• How did you travel to LJI (Plane, train, public transportation, Lyft, Uber, personal vehicle)?
• In the past 14 days, have you had any of the following symptoms:

• Fever greater than 100.4 degrees F
• Cough
• Sore throat
• Difficulty breathing

• In the past 14 days, have you lived with, visited, cared for, or been in a room with someone who is sick, under investigation for or has been confirmed to have COVID-19?
• If your visitor answered “yes” to any of the above health-related questions they will be barred from entering the building, otherwise, discretion will be used in making the entry decision.

ONSITE SCREENING [required]

4. Upon arrival at the building Visitors must contact Operations at the number listed on
the entrance door and wait outside the yellow barriers until given directions by Operations
5. Operations will screen the visitor again over the phone as follows:

• Since last screened by LJI have you had any of the following symptoms:

• Fever greater than 100.4 degrees F
• Cough
• Sore throat
• Difficulty breathing

• Since last screened by LJI have you lived with, visited, cared for, or been in a room with someone who is sick, under investigation for or has been confirmed to have COVID-19?
• Operations will directly notify the LJI contact of approval and allow access to the building
• In some cases, we may require that the Visitor must be escorted at ALL times by the LJI Visitor Contact

Visitors must follow the LJI 6’ Distance and Hand/Frequently Touched Surfaces Disinfection rules. If this is impossible due to work being done such as multi-person lifting then the Visitor’s LJI contact must be notified and a safe solution approved.

Face covering and gloves must be provided by the Visitor/Contractor/Service Provider and be worn at all times.

Any Visitor/Contractor/Service provider exhibiting signs of sickness will be asked to leave the building.

At no point in time is anyone other than Operations allowed to grant access to a visitor.
